How to add a standard image upload dialog to a Wordpress page?

Hello.
I want to create a page template where you can place a form with the ability for the user to upload an image. I believe that a standard Vodpress guevina for an upload would look organic. I even know that this is done using plupload. But I can't figure out how to implement it.
Help with advice or a piece of code, please.
