How reliable are SIM chips really?

Question:
Has anyone used SIM chips instead of SIM cards in their projects, preferably reaching mass production, and has anyone encountered reliability problems? If so, how were the problems solved?
I have 2 out of 50 tested sim-chips stopped working out of the blue.

It seems to me that this is a question for your chip manufacturer, more than 100 million people in Russia use such chips, and I think their reliability is very high.

The fact of the matter is that the manufacturer claims high reliability (I'm waiting for an official answer of 100 ppm). The soldering was carried out in accordance with the temperature recommendations. The wiring is correct - confirmed by the manufacturer of SIM modules. And sim chips die. I would like to predict / accept to reduce failure.
