How not to get terrible traffic quality from google display network?

I'm launching advertising in the GCM from Google, I have a couple of questions:

Why does most of the traffic come with some kind of huge CTR under 100%?
I block such sites in batches, but they go on forever

How to avoid a situation where 80% of traffic goes to 1 site, where there are hundreds of clicks with 100% bounce?
As an example, today there were 120 clicks for 2 rubles per hour with zero quality of site viewing, the clicks ended only when the entire budget was consumed.

What I did:
Selected the audience I needed based on intent
Set a minus 85% for mobile traffic
Removed everything that could be related to games from
topics Disabled impressions on parked domains and other strange places
Narrowed age targeting very much
Added all applications (100 with something) to exceptions
Added over 5000 sites to exceptions

And still, all I get is traffic with a CTR under 100% or banal clicks with refusals.
Perhaps I'm missing something? How to get normal traffic from kms?
