How does the Apple payout process work?

At the beginning of November 2014, I placed the application in the App Store, by the end of the month there were already more than $100 in profit. It's been two months and Apple still hasn't sent me the money. Do I need to submit any application for withdrawal? How is the withdrawal process in general?
Bank Info lists a Russian bank.

Payment will be when $150 or more is typed, then the Next Payment Date will appear in Payments and Financial Reports.
