Does it just scan the entire Internet and all ports all the time? Or how then?

why "all the time"?
Regularly at a certain interval.
Chinese bots with the Internet closed from everyone manage to spam / brute / scan the entire Internet.
If we exclude any known hosting IPs from scanning, etc.
There will be much less than the entire Internet.
