How does getmac find out the remote mac address of a machine?

So I'm wondering how she does it, if the nearest mac-address is the address of your router.

in the local network, machines communicate by mac addresses, of course, each knows the addressee's mac after the first access to it, at least by ping , the
mac outside the local network cannot be found out: there is no practical sense in this

nearest address is the address of any machine in the broadcast domain, not just the address of the router.
