How do I get a Windows Market Age Rating?

Hello. It so happened that most of my applications in the Windows market began to be deleted because they do not have an age rating. How can you get it? Of the 10 sites, I figured out only one (US estimate) and I did not find with the rest where they have an online submission of the application. How do I get a Windows Market Age Rating?

Do you have an app or game? For our application, everything was decided by a competent user agreement and a checkbox that the user has read and agrees with it. 3 or 4 weeks lost because of such a trifle.
The problem was that we have an application for the age category 3 + (Windows 8 App), and according to their (American) laws, children under 13 cannot have their own personal mailbox at all.
