Most likely, your token was stolen. Check your PC with an antivirus and change your password. If you haven't enabled two-factor authentication yet, do so. And in general - be less gullible. If someone throws you a file, check it, and if you are not sure about it, do not open/run it. If some site asks for a password when logging in via OAuth (for example, some site asks for Discord's password when trying to log in via Discord) - log in to the discord site itself and double-check the page again - if it still asks for a password - it is most likely a phishing site.

Changed nickname and put the same avatar. Well, or they really hijacked the session - the discord is full of holes like a sieve, there is nothing to be surprised at.
