I advise you to group by hand. That will be the best. Use filters, stopwords, group analyzes, etc. in the word jar (I don’t remember what tools it has, but it definitely has it in CC). When compiling clusters, rely on logic and competitors.

Let's discuss. Key: "acute migraine." Why is he? What does the user hope to get by accessing this key? This is too general a request - you do not understand what he wants to see - a description of the disease, buy medicine or how to treat it. You need to look at the issue and decide if there are sites similar to yours. Depending on this, leave or delete the key.
