How are landing tests screwed up?

Hello. How are these tests screwed on?
This test is from the site of the designer. It is possible to copy it, but I think it's better to do the same from scratch, copying the html and css code, only changing the scripts, in the worst case. Are there plugins or online generators to do the same?

In order for the data from the form to be processed and the output to be a certain result, PHP is used on the server. Javascript only validates the form fields (matching the given conditions). "copying html and css code, only changing scripts" will not work. To process the form, you need a php file with a handler.
