There is a review layout, there is a review form layout.
Which WP plugin with reviews supports custom output of both the reviews themselves and the form for submitting them?
Required fields: first name, last name, subject, review, photo.
Reviews are intended for a separate page and output on the main page.

It depends where you will have reviews - one page with reviews for sites, or reviews for products or something else.
