Duplication in the document log?

A document is duplicated in the document journal, but only with a specific counterparty. When I open a separate form of the list of the document Purchase of services and other errors, then there is only one document. What is causing this error and how to remove it?

Most likely, a poorly constructed dynamic list query that outputs data for the log.

Is it really the same document? If you mark one for deletion, then the second is also marked? If yes, and you have a standard log (without editing the query), then it looks like an index error - try running reindexing (testing and fixing in the configurator).
