Most likely this is some kind of virus that registered itself in the startup or scheduler. A very strange way, a parody of a system application.
The profiler clearly tells you that it copies data almost all the time to a temporary table, which is also dumped on disk - doubly bad.
I didn't understand indexes, but obviously it's not good to make such large offsets for the limit, why - it is written here:
