Creating a test?

My sister works as a psychologist in some center, she was assigned to develop a test for patients of this center. She successfully created a test, and now she was instructed to "translate to an electronic version"; she asked me to help her with this, but I am like her layman. It is necessary to create a test that will be easy to open on any computer without Internet access, those formats are exe. There are about 40 questions and several answers in the test, but no evaluation reports should appear at the end; after all, these are patients of a psychological center, and it is necessary that they do not find out about the results of their testing, but the psychologists / managers of the center find out. Stupidly ask questions and correct answers in the source code. Send the results to the server of your center with any suitable library.
(Actually, if you are a layman, then in vain you took it up. And most likely, trying to create the .exe you need while dealing with the environment along the way, you will not succeed. Look for a ready-made solution.)
One more thing. There is an inconsistency in your question. Tests should be offline, without internet access, right? But how will the information go to the center? There is no Internet ...

You can create a database of questions and answers and make a program. Let him take questions and answers. For example, you cook the entire result in Json and send it to the server. Nothing complicated.
