Articles / studies on the topic of compiling interfaces (questionnaires) for employees?

Hello, maybe someone came across some good articles/studies that would tell you how to better compose survey interfaces (several pages that contain input fields, radio groups, checkboxes, etc.).

Questionnaires are filled out by employees with information from customers.

I would like to understand how best to compose such questionnaires (how many elements per 1 page, how to combine, etc.)

This is a whole science (UI \ UX, usability), it cannot be described in one article.

In your case, this is secondary.
It is more important not to step on such a rake: the client gives an answer, and the questionnaire does not provide for such an answer option. Always leave the "other" option, even if it seems that there is no "other", people can be inventive.
Leave the opportunity to return to previous pages and correct answers; people sometimes make mistakes and people sometimes want to correct mistakes; unless it is a quiz, then correction should not be prevented.
how best to design questionnaires, in what order to put the fields
The first two or three questions are not very important, for "warming up"; it would be nice to leave the answer in free form for the final, if there are such questions at all; avoid several complex questions in a row, it is better to alternate complex questions with simple ones (but there should still be some logic in the sequence of questions). This is psychology.
Take a specific list of questions and show a person familiar with psychology ask for any advice on that specific list. General rules may not work in particular cases. Psychology is not an exact science.
