Additional fields in prestashop 1.5.6?

Hello everyone, I installed this engine on my head, but there is no sufficient knowledge of php. Required in standard feedback (ticket system). Add the field "phone number" and "Name". So that it is displayed in the admin panel in the message and in the letter sent to the mail. or at least already as a normal feedback can be screwed to the presta?
I ask for at least some tips, they are silent on the off-line forum.
